A number of things could have gone wrong:

• The URL you provided didn't belong to the repository you were tested.
• The URL you provided was not an item page
• The item page couldn't be retrieved within the connection timeout limit of 5 seconds.
• The Item you provided didn't expose a single meta citation tag in its HTML head
